Prevalence is defined as "the total number of cases of a disease existing in a population at a specific point in time".

Information on coronary heart disease (CHD) prevalence can be obtained from a number of sources such as the Download pdf file Scottish Health Survey report on Cardiovascular Disease 2003, chapter 9 of the 2009 Scottish Health Survey (volume 1) and the Quality and Outcomes Framework (QOF). Both data sources (Scottish Health Survey and QOF) provide high level estimates of prevalence.

To get more detailed estimates by age and sex, ISD Scotland's CHD and Stroke Programme used hospital discharge data (SMR01) to identify patients treated in Scottish hospitals with a CHD diagnosis. These records were linked to the national Community Health Index (CHI) and the resulting data forms a register of patients still alive with details of hospital treatment - e.g. history of angina, myocardial infarction, coronary artery bypass or angioplasty, together with details of age, sex, area deprivation score and which GP practice and Community Health Partnership (CHP) to which they belong.

There may be valid differences between the prevalence estimates from these different sources because of their varying methodologies. For example, patients with angina may be diagnosed in cardiology outpatient clinics without having had an inpatient or day case admission. These patients would be excluded from the hospital-admission-based calculation but included in the QOF-based calculation because of their presence on the general practice heart disease register.

Although Table PV1 shows prevalence estimates and crude rates at CHP and health board level only, the data are available at GP practice level for any practice that wishes this. The register data may provide a useful for check for practices against their Quality Outcomes Framework (QOF) registers.

Key Points (from February 2011 release):

  • The estimated prevalence of coronary heart disease based on admission to hospital is 3.3% of the Scottish population. Prevalence is higher in males (4.2%) than in females (2.5%) and is strongly related to age.
  • An estimated 16% of the Scottish population aged 75+ is living with coronary heart disease (CHD). In some, more deprived, community health partnerships around 25% of men aged 75+ have CHD.
  • The highest all-ages prevalence rate is 4.6% in Inverclyde Community Health Partnership (CHP) for males and females combined. This is double the rate in West Glasgow (2.3%), the CHP with the lowest all-ages prevalence.
  • The effect of deprivation on prevalence amongst the younger population can be seen in the 45-64 age group. The prevalence rate in males aged 45-64 in North Glasgow (8.5%) is more than double that of the rate in Shetland CHP (4.0%). For females aged 45-64 the prevalence rate in North Glasgow CHP (5.3%) is nearly five times that in Orkney CHP (1.1%).
